INFORMATION FOR CONTRIBUTORS

Note:  Accepted manuscripts are kept in the Editorial Office for one (1) year and discarded following publication.  Papers out for revision will be discarded after one year if there is no contact with the Editorial Office or activity with the revision.  Rejected papers are returned to authors.

CONTRIBUTIONS: Original typed paper and two copies, not carbon copies, with three sets of illustrations, as well as requests for exchanges and book reviews, should be forwarded prepaid at the author's risk to the Editor of THE AMERICAN SURGEON:

    J. David Richardson, M.D.
    Editor, THE AMERICAN SURGEON
    Department of Surgery
    University of Louisville
550 South Jackson Street, 2nd Floor ACB
    Louisville, KY 40202
    Fax - 502/852-8915
    E-Mail - amsurg@sesc.org

The corresponding author's name, address, telephone number, and FAX number should appear on the title page of the manuscript.

PREPARATION OF MANUSCRIPTS: Manuscripts, including references, must be typed double spaced on one side of the paper using 1” margins. A CD must accompany an original and 2 copies of the paper. While every effort will be made to guard against loss, it is advised that authors retain copies of manuscripts submitted. All pages should be numbered. Numbers one to ten should be spelled out, except when used for all units of measurement (time, dimensions, degrees, weight, volume, dosage, etc.), percentage, and decimals; for numbers above ten, numerals should be used. Dorland's Medical Dictionary and Webster's International Dictionary may be used as standard references. Scientific names for drugs should be used when possible. Copyright or trade names of drugs should be capitalized. Units of measure, e.g. dosage, should be expressed in the metric system. Temperature should be expressed in degrees centigrade.

The corresponding author's name, address, telephone number, FAX number, and e-mail address should appear on the title page of the manuscript. A Copyright Release Form with signatures from all authors listed on the manuscript must accompany the manuscript. If the release form does not accompany the manuscript, the manuscript will not be considered for publication until the form is received in the Editorial Office.

An Abstract of 200 words or less should be submitted to run at the beginning of the article, rather than a summary at the end.

PREPARATION OF ILLUSTRATIONS: Black and white illustrations will be reproduced free of charge, but the publisher reserves the right to establish a reasonable limit upon the number supplied. Color illustrations will be published at the author's expense. For one color photo, the fee is $775.00. The cost graduates with each additional photo. Glossy prints of photos and illustrations should accompany each copy of the manuscript, and they can be on the CD as well. Line drawings (2 sets) should be black on white paper with lettering in black ink, large enough to be readable after necessary reduction.

Large or bulky illustrations should be accompanied by smaller glossy reproductions of the same to facilitate circulation among the judging members of the Editorial Board. Illustrations should be numbered, the tops indicated, and the author's name and the title of the article, in brief, should appear on the back. A separate typewritten sheet of legends for the illustrations should be supplied.

REFERENCES: Number references consecutively in the order in which they are first mentioned in the text. In the double spaced listing at the end of the manuscript, use the form of references adopted by the U.S. National Library of Medicine and used in INDEX MEDICUS. For journals: last name, initials (for all authors); article title (INDEX MEDICUS abbreviation or spelled out), year, volume number, inclusive pages. For books: Last name, initials (for all authors and/or editors); book title, edition (if more than one); location and name of publisher, year of publication; page number(s) if needed to direct readers to a specific section.

TABLES: Tables should be double spaced on separate sheets, numbered, and cited in the text of the article. Abbreviations should be kept to a minimum or appropriately annotated.

REGARDING OLD MANUSCRIPTS: Accepted manuscripts are kept in the Editorial Office for one year and

discarded following publication. Papers out for revision will be discarded after one year if there is no contact with the Editorial Office or activity with the revision. Rejected papers are returned to authors.

BOOK REVIEWS: Any books sent to the Editorial Office for review will not be returned to sender.

LETTERS TO THE EDITOR: Any letter sent to the Editorial Office will be considered for publication. A hard copy must be accompanied by a CD.

REVISED MANUSCRIPTS

Authors are required to submit a CD of the final version of their manuscripts along with the typed revised manuscript. Be sure to turn off tracking notes or comments. Please be sure that the file you send is the most recent version of the manuscript, and that it matches the printed copy that was accepted for publication. The file should contain all parts of the manuscript in one file. Mathematics and tabular material, however, will be processed in the traditional manner and may be excluded from the electronic file.

NOTE: The journal does not assume responsibility for errors in conversion of customized software, newly released software, and special characters.

CDs must have a label on the outside with the journal name, the manuscript number, the senior author's name, and the name of the computer file. To ensure that your CD is processed efficiently, provide the following information: name used to access paper on CD; name of computer used (e.g., IBM/PS2); word processing program and version (e.g., Microsoft Office Word 2003); special characters used in the file (e.g. Greek, math symbols); manuscript number; corresponding author; telephone; and FAX numbers. For electronic digital figures: each figure should be saved in its own file; files should be either TIF, EPS, or JPG; each file should be named according to its figure number and format (e.g., “fig4a.tif”); figures must not be embedded in a word processor or spreadsheet document.
