ON-LINE ABSTRACT SUBMISSION FORM

ANNUAL SCIENTIFIC MEETING FEBRUARY 9-12, 2008

Please read all information regarding submission of an abstract.  If you have not read the guidelines, STOP and do so before filling out this form. Any abstract not conforming to the guidelines will not be accepted and will be returned for the necessary corrections. 

DO NOT rely on the automatic confirmation as abstracts received by Southeastern Surgical Congress.  A written confirmation will be mailed.  If confirmation is not received by July 13, 2007, please contact Southeastern Surgical Congress. 

DEADLINE FOR ABSTRACTS IS JULY 5, 2007    

IMPORTANT NOTICE

If, when submitting your abstract, you receive ANY KIND of ERROR MESSAGE,   take the information out of the boxes and put them into a word document in the same order and send it as an attachment via e-mail to sesc@sesc.org.  Do not send the information in the boxes. Your subject line in the e-mail should be "Abstract Submission."

Title - Watch for spelling. How the title is presented here is how it will be printed in meeting materials.


Names of Author(s), Medical School or Hospital Address, and City/State


Authors' initials precede the last names. Include each author's highest degree, but omit 
FACS and all punctuation within names. Example: JD Doe MD; SM Smith PhD, and 
DL Jones MD. Name of medical school or hospital with address, city and state must be 
included.

Abstract - Tables and graphs MAY NOT be included in the abstract.


No reference to the authors or the institution should appear within the body of the abstract.

Your submission acknowledges these general understandings:

1.   That this material will not be presented elsewhere if accepted for presentation at The Southeastern 
Surgical Congress Scientific Meeting.

2.    That the Editorial Board of "The American Surgeon" assumes copyright ownership and has the
right of first refusal for publication of the material.  Acceptance for paper presentation on the Program
section of the Scientific Meeting obliges you to the submission of an original manuscript for consideration
by "The American Surgeon" at the time of the meeting. Failure to do so may result in sanctions against 
future abstract submissions.

3.   Presenters/authors are responsible for:
      a.    completing the registration form and paying appropriate fees
      b.    all travel expenses associated with attending the meeting.

4.    If abstract is offered a Poster Position and that Position is accepted, the presenters/authors under-
       stand that their attendance is required on Sunday and Monday of the Scientific Meeting. Failure to do
       so may result in sanctions against future abstract submissions and will result in a charge for the Poster
       Presentation Board.
